Free Whitening for Life!

At Laser Dental Associates, we want to help you attain and maintain the smile you have always wanted. So in keeping with that goal, we have introduced our new Free Whitening for Life program.

We will provide you with free dental bleaching kits for life. In order to take advantage of this program, potentially saving you thousands of dollars, the following rules apply:

* One offer per patient

* Patient must adhere to recall program. Patient must keep recall schedule as our staff prescribes

* Hygiene appointments must be kept within 2 weeks of recommended recall interval (3,4, or 6 months).

* All recommended dental treatment must be completed, at the doctor's discretion, before whitening.

* Incomplete dental treatment voids patient's participation in program.

* Failed or missed appointments for any reason will void patient's participation in program.

* Patient must cancel appointments with 48 hours or more notice for the appointment not to be considered failed.

* A maximum of one whitening kit per every 365 days from initial delivery date of first kit.

* Patients under ate 16 are not eligible.

* Patient (or parent in case of minor) must sign informed consent form prior to receiving kit.

* Kits are non-transferable.
