Being well-prepared for your dental appointment will ensure that your dentist has all the needed information to provide the best possible care for you. It will also help relieve any unnecessary anxiety you may be feeling. At your first appointment, please bring the following items:

* Complete list of medications: herbal, over-the-counter and prescriptions.
* Insurance card/information
* Completed forms from our website
* Please remember to take your antibiotic premedication, if necessary.
* Photo ID
